Your exchange works, but does your data?

Better data for every participant, better care for every person.

Connectivity is already solved by your core platform. What the four of us do is make what comes out of those pipes worth using: a complete, robust record with identities resolved, clinical and social data brought together, and the implementation experience to get it live and keep it running.

We’ve already done this together.

The Chicago Regionwide Community Information Exchange® went live in March 2026, connecting health and social service organizations across Chicago and Cook County under shared governance and real‑time data integration. 4medica, wellconnected and NJII built it together.

Roundtable Session

Closing the “Last Mile” of Interoperability: Building Resilient National‑to‑Local Data Infrastructure

DateWednesday, September 23
Time1:15–2:30 pm ET

As national frameworks such as TEFCA move from policy into implementation, the work of making data usable falls to state and regional infrastructure. This session takes on the practical side of that: data quality, patient matching, governance alignment and regional onboarding.

  • Identify the gaps between national standards and regional implementation that limit usability.
  • Evaluate approaches to data integrity, matching accuracy and de‑duplication.
  • Explore governance models that align with federal frameworks while meeting local needs.
