Reducing preventable hospital readmissions is a top priority for providers and public and private health plans costing billions of dollars annually in the U.S. To prevent readmission, providers must have access to accurate and timely sources of medical histories, current medications, and lifestyle choices. COVID-19 raised awareness of the need to provide New Jersey hospitals with complete and accurate data, making quality data a priority.

The New Jersey Health Information Network (NJHIN), as part of the New Jersey Department of Health, is unique in that is works with all of New Jersey’s 71 hospitals to collect and share data. Sharing accurate and timely data among disparate systems is difficult, but with their use of select technologies, they have been able to correctly match patients and provide physicians with correct data to improve the hospital readmission statistics in New Jersey.
